Public Health Impact of the Adjuvanted RSVPreF3 Vaccine for Respiratory Syncytial Virus Prevention Among Older Adults in the United States
This study uses a Markov model to estimate that vaccinating 56.7 million US adults aged โฅ60 with the adjuvanted RSVPreF3 vaccine could prevent nearly 3 million symptomatic RSV cases and over 16,000 deaths over three years.

Respiratory syncytial virus (RSV) is a significant cause of lower respiratory tract disease in older adults, leading to considerable morbidity and mortality. This study evaluates the public health impact of the adjuvanted RSVPreF3 vaccine in US adults aged โฅ60 years, aiming to quantify reductions in RSV-related clinical outcomes and healthcare resource use.

A static, multi-cohort Markov model was developed to simulate RSV-related outcomes over a 3-year horizon, comparing scenarios with and without one-time RSV vaccination. The base-case analysis assumed vaccination coverage equal to that of influenza vaccines. Key epidemiological and vaccine inputs were derived from published literature and phase 3 clinical trial results for the adjuvanted RSVPreF3 vaccine. Outcomes included symptomatic RSV acute respiratory illness (RSV-ARI) cases (upper and lower respiratory tract disease), pneumonia complications, mortality, and healthcare resource use (hospitalizations, emergency department visits, outpatient visits, and antibiotic prescriptions). Sensitivity analyses were conducted to assess robustness.
